Federal and state laws require hospitals, local registrars and county clerks to give unmarried parents the chance to voluntarily acknowledge paternity as soon as possible after their baby is born and to give parents a verbal and written explanation of their rights and responsibilities.

You should not sign the acknowledgement form until you receive your written and verbal explanation.

A verbal explanation of parents' rights and responsibilities when establishing paternity is available by calling:

The Child Support Customer Service Call Center:

  • Is available 24 hours a day, 7 days a week
  • Is a toll-free call
  • Serves the entire state

Parents who live outside Illinois and have a child living in Illinois can receive an audio or video tape with their rights and responsibilities by calling 312-793-8213.
