The Child Support Application is available for download in Portable
Document Format (PDF) and is now provided in a format that
can be filled out on your computer. You will need to download
and install the Acrobat Reader, which is available from Adobe's
Web site, on your computer to view or print these files. Click
on the Adobe® Icon for the free
Download. In order to use the fillable features,
please note the following:
- You cannot submit the application on the Internet. Rather,
the fillable application will enable you to fill out the application,
and print it out, all on your local computer.
- You may not be able to save this file on your own computer.
We recommend that you fill out a page of the form, then print
it prior to quitting Acrobat Reader.
- In order to make use of all of the features of our fillable
forms, you should use version 5 of the free Adobe Acrobat Reader.
